WINTER TRY-OUTS

Reminder :all paperwork including emergency contact and physical examination forms as well as Concussion Testing (if necessitated) must be completed in order to participate in try-outs.

PHYSICAL good for one full year of participation

EMERGENCY CONTACT

CHANGE IN MEDICAL STATUS-must be completed in the event that medical status has changed with-in the one year period since the physical evaluation form was completed

WINTER SPORTS PHYSICALS
In Conjunction with Liberty High School, My Care Express will be conducting discounted physical examinations for student athletes beginning this winter season. Cost will be $40 and for each physical My Care Express will donate $10 to the school's athletic program in memory of Emily Burke, Liberty Class of 2010.
